This form is to be used by graduate students to request enrollment into the Internship required for their program. Students cannot enroll themselves.

Field Education Internship Requirements:

• All MTS internships must be done in collaboration with a Christian organization (It could be a church, non-profit, hospital, jail, etc. If you aren’t sure if it qualifies: reach out to Daniel Thorpe (daniel.thorpe@moody.edu). 
• There must be an individual within the organization who is willing and capable of serving as a site supervisor and a mentor for the Moody student. They will submit an evaluation upon completion of the internship. It is the student's responsibility to select a ministry site and site supervisor.
• All 3 credit internships should include a minimum of 96 hours of internship-related work over the course of an 8-week session.
• Students in an MA program must have completed at least 18 credits of their degree program, including any necessary prerequisites.
• Students in an MDiv program most have completed at least 60 credits of their degree program, including any necessary prerequisites.

Procedure:

1. The student completes this form no earlier than 2-weeks prior to open enrollment for the term requested to generate their request for enrollment (Academic Calendar) and no later than one week after the enrollment deadline for the session.
2. Once submitted, this form will generate an email request that is sent Daniel Thorpe, the Field Education Instructor, for review.  
3. After review by the Field Education Instructor, it will be forwarded to your site supervisor for approval, and must be returned to Daniel Thorpe (daniel.thorpe@moody.edu) for final verification.
4. Once it is verified, the Field Education Instructor will forward the application to Academic Records to process your registration.
