Undergraduate Program Extension Request

Students who need additional time to complete their certificate or degree program must submit the program extension appeal prior to the final semester of their program time limit.

Approved Extensions- Students will be notified of the terms of their extension. They will be billed $50.00 for every additional semester granted. Failure to complete all requirements by the approved program completion date will result in program dismissal.

Denied Extensions- Students will be required to complete all requirements by the original program time limit. Failure to do so will result in program dismissal.

If granted an extension, I will complete the requirements listed below during the semester indicated:

Payment Information

You will be charged $50 for an extension of 1 semester.

You will be charged $100 for an extension of 2 semesters.

You will be charged $150 for an extension of 3 semesters.

You will be charged $200 for an extension of 4 semesters.

You will be charged $250 for an extension of 5 semesters.

You will be charged $300 for an extension of 6 semesters.

The above fee will be added to your student bill if your request is approved.

If accepted, your program will extend to the term you provided. and your graduation date will be updated. This web form serves as a binding document to your program extension. By typing your name below, you agree to these terms.

If payment information was provided, you authorize Moody Bible Institute to charge the credit card number above for the extension you are requesting.
