Requests are subject to the approval of Academic Records, particularly for students in their junior or senior year.
F-1 Visa Students: You must notify the international student office that you are changing your program (iso@moody.edu)
Students seeking to leave or enter either the Elementary Education or one of the Music programs must first obtain approval from Dr. Lisa Smith or Dr. Jori Jennings, respectively.
This completed form must be submitted to the Academic Records office by the first day of the student’s first course of the semester in order for the program change to apply to that semester’s financial aid.
This form may also be used to request an interdisciplinary major. Interdisciplinary electives may be fulfilled by any courses for which you meet the prerequisites, and may be decided after changing your major.
Students switching from a Chicago to an online program must notify housing@moody.edu to inform them of their departure from campus.

Spokane students who wish to transfer to a Chicago campus program must reapply through the Office of Admissions (admissions@moody.edu).
Upon submission of this request, the Spokane Program Manager and your academic advisor will be notified of the request to change. It is expected that you have already or will soon meet with these individuals.

This form may also be used to request a change to a student’s current online degree program concentration.
Online Bachelors, Associates, or Certificate students must reapply through the Office of Admission in order to change to a Chicago program. Students enrolled in a non-degree program must apply through the Office of Admissions to be admitted into a certificate or degree program.
