All requests will be forwarded to Dr. Cook for approval. If granted, you will receive confirmation via email.
This form is only for use by students who have been admitted to the DMin program. Students in all other programs must apply through the office of admissions to change to the DMin program.

- Fall semester - August 1 - September 30
- Spring semester - January 1 - February 28
