Request for Change of DMin Concentration

Please use this form to change your DMin concentration.

  • All requests will be forwarded to Dr. Cook for approval. If granted, you will receive confirmation via email.
  • This form is only for use by students who have been admitted to the DMin program. Students in all other programs must apply through the office of admissions to change to the DMin program.
  • This completed form must be submitted by according to the dates below:
    • Fall semester - August 1 - September 30
    • Spring semester - January 1 - February 28
    • Summer semester - May 1 - May 31
