This form allows a student to petition to be added to a course after the registration deadline has passed. Please submit one form per course. Submission of this form does not guarantee the request will be approved.

Is this an online asynchronous 8 or 16-week course?

Policy and Agreement:

Chances for academic success decrease significantly when students are enrolled late into online courses. This is because late registration can lead to:

  • Late or delayed access to Moody's online classrooms in Canvas
  • Late receipt of required course materials or books for the course
  • Poor performance due to missed early assignments and course introduction
  • Late financial aid processing or financial holds for upcoming sessions

By filling out this form, I understand and agree to the following late-add policies:

  • I will be charged a non-refundable $50 late add fee per course which will be added to my student bill
  • Any attendance and participation requirements are at the discretion of the course instructor. 
  • I will not be given any special exceptions for the submission of late course work - all course work is subject to the same submission deadline as all other students in the course
  • I will not be given any special exceptions for the drop/refund policy (see the academic catalog for more information)
  • I will not be given any special exceptions to the grade penalties given for late course drops (see the academic catalog for more information).
  • Payment arrangements must be made within 24 hours from the time I am registered for the course or I will be dropped from the course.
  • I will not hold MBI responsible for late access to Canvas and am completely and solely responsible to obtain required course materials.
  • I understand that if I have not paid or enrolled in a payment plan within 24 hours of being enrolled, I may be dropped from my upcoming course(s) despite having filled out this form and will not receive a refund on the late-add fee ($50). Tuition and other fees may be issued with a 100% refund if you are dropped for non-payment.
  • Requests by newly admitted students will be automatically denied
  • Requests by students on academic probation will be automatically denied

All accompanying approvals, where applicable, must accompany this form and the course must be available. If course is not available, this request will be automatically denied and you will not be charged a late add fee.

The course must count towards your academic program. Courses that do not count towards your official academic program will not be added and the request will be denied and you will not be charged a late add fee.

Submission of this form does not guarantee that your request will be approved. If your request is denied, you will not be charged a late fee.
